Comments:
An unannounced monitoring visit was conducted today between the hours of 2:10pm through 4:15pm with the provider on site. There were 3 children (7 points) with the provider. A sample of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, nap time, snack, painting and hand washing. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain the minimum items required. Evidence: 1. The first aid kit was missing the first aid instruction manual, one triangular bandage and gloves.

Plan of Correction: I will get a new manual at my MAT training. I will buy the triangular bandage and gloves today.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the caregiver did not ensure the children's hands were washed with liquid soap and warm water before eating. Evidence: 1. One child went into the kitchen and got a bag of cereal and walked into the family room. Two children were observed eating cereal out of a bag and did not wash their hands prior to eating.

Plan of Correction: I will make sure they wash their hands.
