An unannounced renewal inspection was conducted today between the hours of 9:00 am through 10:30. There were a total of 6 children (14 points) with 1 caregiver. A total of 3 children's files and 1staff file were reviewed. The children were engaged in the following: breakfast, coloring, cars, and building blocks. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider has not obtained documentation of a physical examination prior to or within 30 days after the first day of attendance. Evidence: Child #1, enrolled 4/19/17 does not have documentation of a physical examination within 30 days after the child's first day of attendance. Plan of Correction: I will ask the parents for the physical examination.
Standard #: 22VAC40-111-320-G Description: Based on provider interview, the wood burning fireplace has not be inspected annually. Evidence: The provider stated that the fire place has not been inspected annually. Plan of Correction: I will get the fireplace inspected.
