54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced Renewal study was conducted today beginning at 12:50pm. 7 children (21pts) were directly supervised by 2 caregivers. Ratios were in compliance. The physical space, Emergency Preparedness Plan, 5 children's records, provider/household member records, evacuation drills and emergency supplies were reviewed. Required documents were posted. The space was clean, organized and contained an abundance of age appropriate materials. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, the Provider did not obtain written authorization for emergency medical care for 2 children's records. Evidence: 2 out of 5 records reviewed did not have the required information signed. Child #1 and Child #2 were missing the information and parent's signature for emergency authorization. Plan of Correction: I will have them sign it today.
