54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 9:45am-11:15am. Upon arrival there were 4 children in care, totaling 10 points with one caregiver. All four children's records were reviewed and all household/caregiver records were also reviewed. The licensee does not provide medication. Children were observed painting. One child was observed sleeping.
Standard #: 22VAC40-111-430-D Description: Based on observation the licensee failed to ensure guardrails on the side of stairs were constructed to prevent a child from climbing over, crawling or falling through, or becoming entrapped. Evidence: The front entrance of the home had 4 steps leading to the front door. The side rails were more than 4" wide and several of the slats were broken and hanging off the rail. Plan of Correction: The licensee will repair the railings on the front porch.
Standard #: 22VAC40-111-520-C Description: Based on observation the licensee failed to ensure all requirements were met when using cribs. Evidence: A baby was observed sleeping in a mesh sided play pen. Plan of Correction: The licensee has a portable crib. The play pen will no longer be used.
