Technical Assistance:
**Discussed with the provider that the resilient material is too large to be used for the playground, and there were several pieces that were 3-5 inches in length with potentially hazardous ends (points). Please ensure that when you obtain more resilient material, that you choose a material that is better suited for use on playgrounds for young children. **Did not inspect the upper level of the home, due to renovation work occurring during the inspection. Will inspect at the next visit.

Comments:
An unannounced monitoring/renewal inspection was conducted today from 11:40am-1:00pm. There were 8 children (28 points) directly supervised by 3 staff. The home, 3 staff records, 3 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, napping and eating lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-60-B
Description: Based on review, the provider failed to ensure that children's records contained the required information. Evidence: Reviewed 3 children's records and found that CH3 did not have documentation of a complete address for the second emergency contact.

Plan of Correction: I will ask the parent today when they come to pick up.

Standard #: 22VAC40-111-170-A
Description: Based on review, the provider failed to ensure that they obtain from each caregiver and each adult household member a current Report of Tuberculosis Screening form. Evidence: Reviewed 3 staff records and 1 household member's record and found that HM1 did not have documentation of a TB screening.

Plan of Correction: I will get them to do it next week.
