- Technical Assistance:
Suggested to lower crib mattress as the infants are getting older. Also discussed caregiver files.
Today an unannounced Monitoring/Renewal inspection was conducted. This visit took place in the morning and early afternoon between 9:00am and 12:oopm. There were 8 children present with the provider and one assistant for a total of 21 points within the supervision guidelines. Positive interactions with the children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles are not labeled with the children's full name and today's date. Evidence: There are three unlabeled bottles in the refrigerator and three unlabeled bottles on the counter top. Plan of Correction: I will label the bottles everyday.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's files were missing the required information. Evidence: 1. Child #1 is missing their medical insurance policy number. 2. Child #2 is missing the address for one of their emergency contacts. 3. Child #3 is missing the phone number and street address for their pediatrician and the medical insurance policy number. Plan of Correction: We will request that information from the parents.
Standard #: 22VAC40-111-280 Description: Based on observation, hazardous substances were not stored in an inaccessible location. Evidence: 1. There as disinfectant spray cleaner stored on a shelf. 2. There are disinfectant wipes stored on the counter top. 3. There is hand sanitizer stored next to the sink. Plan of Correction: We will lock them away.
Standard #: 22VAC40-111-320-B Description: Based on observation, protective covers were not used on unused electrical outlets. Evidence: There is an outlet above the counter top that that does not have a protective cover. Plan of Correction: We unplug them when we use them. I will cover them back up.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review and interview, household members did not have the required Sworn Disclosure statement. Evidence: Caregiver interview confirmed household member #3 does not have a Sworn Disclosure statement. Plan of Correction: We will have the household member complete the form.
