Technical Assistance:
Online training information provided to provider as a resource. Discussion on the new background checks that became effective 7/1/2017. As a reminder, stuffed toys are not permitted in cribs while infants are sleeping.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 12 children (32pts) with Provider and two assistants. Children were observed preparing for naptime. Observation of hand washing was made. All staff and household records were reviewed. Current CPR/Frist Aid certification was verified. Annual training for staff well documented. All evacuation drills and one shelter in place drill has been documented since the last licensing inspection. Staff observed sitting with children that were falling asleep. Positive interactions were observed. The home was well organized, amp;le toys and equipment determined appropriate for the children in care was found and accessible to the children. All topical ointments were labeled and dated. No medications on site. Artwork displayed that had been completed by the children. A total of 5 children's records were reviewed and determined complete. New rails installed for the steps leading to the programming area was observed. The previous violation notice was reviewed and corrective actions were verified. Thank you for your cooperation today. Description: Based on review of staff records, one sworn disclosure was not complete. Evidence: Staff #2 was hired in August 2017, the sworn disclosure completed is not the most current sworn disclosure form required.

Plan of Correction: I will print the new sworn disclosure form and have this staff complete.
