Today an unannounced monitoring visit was conducted with the Provider's assistant between 9:30-11am. There were 5 children with 1 caregiver for a total of 17 points, over the points allowed per caregiver. The children were observed arriving, watching tv, having story-time, and actively engaged in self-selected activities. A complete inspection of the physical plant, children and provider records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation of the bottles, bottles were found without the name and or date. Evidence: 2 bottles were found without the date. Plan of Correction: Caregiver stated that parents will be reminded to add the date.
Standard #: 22VAC40-111-530-B Description: Based on observation, the crib being used did not have a tight-fitting bottom cover. Evidence: Upon arrival, a infant was observed in a crib however the crib with a loose-fitting bottom cover. Plan of Correction: Caregiver stated that fitted sheets were being washed and they were put on cribs by nap time.
Standard #: 22VAC40-111-570-A Description: Based on observation and review of records, the provider did not ensure that the caregiver exceeds the 16 points allowed without needing an additional caregiver. Evidence: Upon arrival, Caregiver was present with 3 children(9points) however 2 more children arrived during this visit and the caregiver was observed alone with 5 children(17points) over the points allowed per caregiver. Plan of Correction: Caregiver stated that the assistant was having car problems today and will be arriving soon. This caused the provider to go over points allowed by one point.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation and interview, the findings of the most recent inspection of the facility was not found posted. Evidence: Upon arrival, the violation notice dated (3/21/17) was not found posted in the family day home. Plan of Correction: Provider stated that the notice was posted. Provider will ensure that all caregivers are aware of its location.
