A SHSI inspection was conducted today with the provider and one assistant. Nine children were in care during the inspection and were observed during free play. The age range of the children was three months to four years old. Inspection time 9:15am-10:45am

Standard #: VENDHOM-000-012-A
Description: Based on interviews with the provider there is no documentation of Pre-Service training. Evidence: There is no documentation of the provider taking the Pre- Service training and assistant A stated they did not compete the training.

Plan of Correction: The assistant will take the course and the provider states they got it however can not find the documentation.

Description: Based on review of the emergency and relocation plan is not complete. Evidence: The emergency relocation plan is missing documentation of reuniting of parents and children.

Plan of Correction: I will update the plan.
