Comments:
An unannounced monitoring inspection was conducted today beginning at 11:25am. 10 children (25 points) were present with 3 caregivers. Ratios were in compliance. The physical space, 5 children records, provider/substitute/household member records, evacuation drills and attendance records were reviewed. Required documents were posted. The space was clean and contained an ample supply of age appropriate materials. Children were observed playing outside and preparing for nap time. Diapering and hand washing procedures were also observed. Caregiver/child interactions were positive. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-120-B
Description: Based on record review, 2 out of 2 substitute provider records were incomplete. Evidence: 2 references were unavailable for review for Caregiver #1 and Caregiver #2.

Plan of Correction: I have them. I will find them and email them to you.

Standard #: 22VAC40-111-200-A
Description: Based on record review, one out of two substitute providers did not have documentation of orientation by the end of the first week of assuming job responsibilities. Evidence: The documentation of orientation for Caregiver #1 was not available for review.

Plan of Correction: The substitute completed the orientation. I will find the document and email it to you.
