An unannounced renewal inspection was conducted with provider and two assistants. There were twelve day care children present totalling 28 points. The children were coloring, playing in housekeeping (making pretend meals), and playing with age appropriate toys. Twelve children files and three household/one staff files were reviewed for items missing from previous inspections - please see Supplemental Summary for missing items. Staff are current in first aid and CPR certification. Standard #: 22VAC40-111-60-B Description: Based on review of twelve children's files, the provider failed to have documentation of two emergency contacts for each child. Evidence: there was only one emergency contact for Child 4 and two are required. Plan of Correction: Provider will ask the parent for a second emergency contact.
Standard #: 22VAC40-111-180-A Description: Based on review of three household files and one staff file, the provider failed to have documentation of a repeat tuberculosis screening or test. Evidence: Staff A did not have documentation of a current tuberculosis screening on file. Plan of Correction: Staff A will contact the physician to get a copy of the tuberculosis screening.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of three household files and one staff files, the provider failed to have repeat sworn disclosure statement for each household member and staff. Household member 1 and Household member 2 do not have repeat sworn disclosure statements. Plan of Correction: Household members will complete a new sworn disclosure statement.
