An unannounced renewal inspection was conducted today from approximately 8:34am-9:50am. There were 8 children (18points) directly supervised by 2 caregivers. A sample of 5 children?s records, 1 household member's record and the provider's record were reviewed. The children were observed during drop off, taking a nap, playing with blocks and playing with dolls. Zero new staff has been hired since the last inspection. Description: Based on children's files reviewed, children's files were incomplete, missing some required information. Evidence: 4 of 5 children's files reviewed were incomplete. Missing information includes: 1) Child A was missing a physicians signature on the comprehensive physical examination. 2) Child B was missing the address of 2 of 2 emergency contacts. 3) Child C was missing the name, address and phone number of the child's doctor. 4) Child D was missing the mother's address

Plan of Correction: I will get the parents to complete the files.

Description: Based on observation, A landline telephone, excluding a cordless or cell phone was not accessible during the family day home's hours of operation. Evidence: At the time of inspection the landline phone being used was a cordless phone requiring electricity to operate.
