Technical Assistance:
Training resource provided to staff for 10 hours of free training online. Discussion on the requirements for cleaning and sanitizing your new diapering mat. A reminder that to properly sanitize one tablespoon of bleach is mixed with one gallon of water and prepared fresh daily. Discussion on the code changes in July regarding Sworn Disclosures and Central Registry checks are current for five years. No current staff who have lived outside of Virginia in the past five years. Please forward your renewal application for the license which expires in January 2018.

An unannounced monitoring inspection was conducted this date. A total of 6 children, (17 pts) with Provider and 1 assistant present. Walk thru of programming area, crib area, bathroom used by children and observation of the diapering area was conducted. Children were observed singing and dancing to a children's video, interacting with staff and playing with blocks and other toys available to them. Fire evacuation, shelter in place drills, CPR and First Aid certifications were reviewed and current. No medications on site. Description: Based on review of children's records, three records were not complete. Evidence: Child # 5's last annual review of emergency contacts was dated 7/20/16. Child # 4's last annual review of emergency contacts was dated 4/15/16 and Child # 6's last annual review of emergency contacts was dated 12/12/16.

Plan of Correction: Provider will have conduct the annual review of these emergency contacts with the parents today.
