- Technical Assistance:
Discussed the implementation of finger printing for all staff and household members. You are reminded that the new Sworn Disclosure statement to include residency outside of Virginia must be used. Central Registry and Sworn Disclosure statements are effective for 5 years. Provider has a total of 5 cribs in use at this time. All five cribs that were on site and are currently being used were evaluated and determined in compliance with the Consumer Product Safety Commission.
An unannounced monitoring inspection was conducted this date. A total of 11 children (30 pts) with Provider and 1 assistant present. Walk thru of physical plant, observation of children playing freely with toys and books, napping and preparation of lunch was made. A total of 7 children's records and staff's background checks, TB screenings and annual training were reviewed. Lunch today is fish sticks and fresh fruit. The programming area was equipped with ample amounts of toys and equipment that were determined age appropriate for the children currently in care. Standard #: 22VAC40-111-100-A Description: Based on child record review, one physical exam was not obtained. Evidence: Child #1 began enrollment on 4/19/17. No physical exam was obtained within 30 days of start date. Plan of Correction: Provider will request a physical exam for this child today.
