Comments:
A Subsidy Health and Safety inspection was conducted on 10/16/17 to review supplemental health and safety requirements. There was no children present during this visit with the provider. The provider stated that the children come on holidays/when school is out and after school. A complete inspection of the physical plant, children and provider records, fire drill log, and emergency procedures were observed during this inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on review and interview with the Provider, the emergency procedures did not include documentation of a reunification plan. Evidence: The emergency plan did not include procedures to reunite children with parent in the event of an emergency.

Plan of Correction: Provider added a reunification plan to the emergency plan during this visit.

Standard #: VENDHOM-000-031-G
Description: Based on observation and interview with Provider, the provider did not have written documentation of a steps to prevent cross contamination. Evidence: Provider provides food to children however she did not have documentation of allergies posted to prevent cross contamination.
