An unannounced monitoring inspection was conducted today between the hours of 8:15 am through 10:00. There were a total of 5 children (18 points) with 2 caregivers. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, books, diapering, and hand washing. Standard #: 22VAC40-111-60-B Description: Based on record review, the provider has not maintained a complete record for each child. Evidence: The following information was missing; Child #2 and #3 are missing a 2nd emergency contact. Child #1, #2 and #3 are missing the medical insurance policy number. Child #3 is missing written parental agreement for emergency medical care. Plan of Correction: I will get the information from the parents.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider has not obtained a report of Tuberculosis screening every 2 years from the date of the initial screening. Evidence: Household #1, TB dated 10/13/15 Household #2, TB dated 10/17/15 Household #3, TB dated 10/6/15 Plan of Correction: WE will get TB screening.
