Comments:
A Subsidy Health and Safety inspection was conducted this date to review supplemental health and safety requirements. At the time of entrance, 5 children were in care with 2 staff members present. Children were observed during toy play, story time, diapering and hand washing. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please contact DSS to cancel your contract with subsidy.

Description: Based on record review and provider interview, the perspective vendor has not completed Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. Evidence: The provider stated that she has not completed the required training.

Plan of Correction: I will cancel my contract with subsidy.

Description: Based on record review and provider interview, caregivers have not completed Virginia Pre-Service Training for Child Care Staff sponsored by the Department of Social Services. Evidence: The provider stated that the assistants have not completed the required training.

Plan of Correction: I will cancel my contract with subsidy.

Description: Based on record review and provider interview, caregivers have not obtained current certification in cardiopulmonary resuscitation (CPR). Evidence: Caregiver #3 was assisting today, the caregiver does not have current CPR.

Plan of Correction: I will cancel my contract with subsidy.

Description: Based on record review and provider interview, caregivers have not obtained current first aid certification. Evidence: Caregiver #3 was assisting today, the caregiver does not have current first aid certification.

Description: Based on a review of emergency plan, the family day home does not have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to a lockdown . Evidence: The provider has not included procedures for a lockdown.

Description: Based on record review , the evacuation and relocation procedures do not include procedures to reunite children with a parent or authorized person designated by the parent to pick up the child. Evidence: The providers emergency plan does not include procedures to reunite children with a parent or authorized person designated by the parent to pick up the child.
