Comments:
An unannounced monitoring visit was conducted today between the hours of 9:00am through 11:15am with the provider on site. There were 9 children (30 points) with the provider and three assistants. A sample of 5 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, Spanish lesson on colors, singing songs in English and Spanish, listening to music and outside time. The home is clean and organized with lots of toys. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles kept in the refrigerator were not labeled with the child's full name and the date. Evidence: 1. There were three bottles in the refrigerator labeled with the child's name on it, but the bottles were not dated.

Plan of Correction: I will date them immediately.

Standard #: 22VAC40-111-60-B
Description: Based on review, 3 of 5 children's records were not complete. Evidence: 1. Child A did not have documentation of the yearly review of the child's emergency contact information as it was last signed on 10/03/2016. 2.. Child B did not have the child's medical insurance policy number. and one parents work place address. 3. Child C did not have one parents work place address.

Plan of Correction: I will send the parents a message today for the information.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation , the findings of the most recent inspection of the facility were not posted. Evidence: 1. The findings of the inspection conducted 06/19/2017 was not posted. The inspection dated 04/06/2016 was posted and the acknowledgement form from the 06/19/2017 inspection was under it. The caregiver stated she never received a copy of the last inspection.

Plan of Correction: I will post the current inspection today.
