An unannounced renewal study was conducted today beginning at 11:05am. 11 children (26 points) were directly supervised by 3 caregivers. Ratios were in compliance. The physical space, caregiver/household member records, 5 children's records, evacuation drills, the menu, the attendance log and Emergency Preparedness Plan were reviewed. All caregivers completed their annual training requirements. The space was clean and organized. Children were observed playing outside and infants were observed waking up from their morning nap. Lunch today was ham and cheese sandwiches with peppers and pears. Milk was served as the drink. Policies were followed and interactions between caregivers and children were very positive. Description: Based on record review, the Provider failed to obtain the parent's signed acknowledgment of the review of emergency information. Evidence: 5 out of 5 records reviewed were overdue for their annual review of emergency information. The last reviews were as follows: Child #1 - 6/14/2016, Child #2 - 1/5/2016, Child #3 - 8/12/2016, Child #4 - 5/1/2016, Child #5 - 8/13/2016

Plan of Correction: I will ask the parents to review the information and sign the document.

Description: Based on record review, the provider did not maintain a complete child's record. Evidence: Child #5 was missing the second page (with the parent's signtaure) of the Policy for the Administration of Medications.
