Inspector and provider discussed maintaining background check documentation.
An unannounced renewal inspection was conducted October 20, 2017 from 9:30am-11:00am. There were 5 children (12 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. Diapering procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. Evidence: The provider has not conducted a shelter in place drill a minimum of twice per year. The provider does not have a documented shelter in place drill for the 2017 year. Plan of Correction: I have a shelter in place. (sic)
Standard #: 22VAC40-111-960-E Description: Based on observation, the provider did not label bottles with the date. Evidence: In the refrigerator, there were 5 bottles labeled with the child's name but not the date. Plan of Correction: Bottles have name and date everyday (sic)
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after first day of attendance. Evidence: Child A does not have documentation of a physical examination. Plan of Correction: Child has physical exam. (sic)
