An unannounced monitoring inspection was conducted today, 10/26/17. There are two children in care totaling four points with one caregiver. The children were busy reading, playing with the available toys and having lunch. Hand washing was also observed. A review of children and staff records took place during the inspection. Should you have any questions please do not hesitate to call. Inspection began: 10:15 am ended 11:45 am.

Standard #: 22VAC40-111-80-A
Description: Based on review of children's records the provider failed to receive all of the required information. Evidence: 1. Child #1 entered care on 8/22/17. There is not proof of age and identity in the file.

Plan of Correction: The provider will have the parent provide this information.

Standard #: 22VAC40-111-350-E
Description: Based on observation the provider failed to maintain the water temperature within the range of 105 F to 120 F. Evidence: 1. The bathroom used by the children has a water temperature of 128 F.

Plan of Correction: The provider will have the water heater adjusted.
