Technical Assistance:
During the inspection, licensing representative discussed with Provider annual staff training requirements. Licensing representative also provided Provider link to obtain model form to track potential substitute provider's hours annually.

A renewal inspection was conducted today from 12:00pm-2:00pm. There were 6 children (14 points) directly supervised by 2 staff. The home, 2 staff records, children's records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping and bottle feeding. There was an abundant supply of books, toys and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-750-A
Description: Based on observation, topical skin products did not meet the stated requirements. Evidence: 4 diaper ointments and 1 body cream were not labeled with the children's names.

Plan of Correction: I did put Kids name on it [sic]

Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not update the emergency plan at least annually. Evidence: The emergency plan has not been updated since licensure in June 2016.

Description: Based on review of emergency response drills, not shelter-in-place drills have been practiced. Evidence: There was no documentation of shelter-in-place drills practiced since August 2016.

Description: Based on a review of records, 1 children's record was not complete. Evidence: The record for Child #1 was not updated annually.

Plan of Correction: I did talk to Parents to summit Kids annual record [sic]

Description: Based on record review, not all children had a record of updated immunizations for children under two years of age. Evidence: 1. Child 2 (DOB 6/15/15) has the most recent immunizations dated 11/10/2016. 2. Child # (DOB 5/23/14) has the most recent immunization update dated 8/24/15.

Description: Based on a review of records, not all household members had a Report of Tuberculosis Screening form on file. Evidence: Household Member #3 [who also reported being a volunteer] was missing TB results.

Plan of Correction: I will make appointment with doctor for Her [sic]

Standard #: 22VAC40-111-200-A
Description: Based on review of caregiver records, 1 caregiver was missing documentation of the required training. Evidence: Caregiver #2 was missing documentation of orientation training.

Plan of Correction: she will take Traning with in week Today October 24th i gave orientation class to my assistant [sic]

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored inaccessible to children. Evidence: 1. A bottle of hand sanitizer was observed in a hanging door storage accessible to children. 2. A can of disinfectant spray was observed in an unlocked lower cabinet in the children's area.

Plan of Correction: I already removed sanitizer from the kids hanging door I already removed the disinfect spray from cabinet

Standard #: 22VAC40-111-530-F
Description: Based on observation, soft bedding was observed in a crib with an infant. Evidence: An 11 month old infant was observed in a crib with two plush blankets wrapped around them and a third blanket used as a pillow underneath their head.

Plan of Correction: I will never use blanket for infants
