Technical Assistance:
Reviewed all current and upcoming changes to background check procedures. All future information regarding background checks will only be communicated via email.

Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 11 children, totaling 27 points. Children's records, new caregiver records, emergency preparedness items, the physical plant, children's medications, and administrative documents were inspected today. It was reported to the Inspector that one child's medication was on site today. The children were observed playing with toys and puzzles, practicing color and shape recognition skills, playing outside, and eating lunch. The children were served organic cheese/spinach/chicken quesadillas, bananas, and milk for lunch today. This inspection was completed between approximately 9:45 am and 11:20 am. Violations:
Standard #: 22VAC40-111-920
Description: Based on observations, it was determined that the menu for the current one-week period was not posted. Evidence: The provider has not posted the menu for the current week.

Plan of Correction: I will remember to post the menu.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: Assistant caregiver record A does not include emergency contact information and the documentation of two completed reference checks.

Plan of Correction: I will have her complete the emergency contact sheet today. I called the reference checks but forgot to write it down. I will put the documentation in her file.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two children's records were not complete. Evidence: 1. Child record #1 does not include the mother's work phone number. 2. Child record #3 does not include the parent's medical insurance policy number.

Plan of Correction: I will ask each parent for the missing information.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that the provider did not obtain a repeat TB Health Screening for one assistant caregiver within 2 years from the most recently completed screening. Evidence: The TB Health Screening for assistant caregiver B expired on 8/21/17.

Plan of Correction: I will make sure she gets a repeat TB screening.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training records, it was determined that the provider did not document the completion of Orientation Training for one assistant caregiver. Evidence: There is no documentation of completed Orientation Training for assistant caregiver A.

Plan of Correction: I did the orientation with her. I will complete the documentation and put it in her file.
