Technical Assistance:
During inspection, licensing representative discussed with provider requirements for household member who will be turning 14 years old at the end of the year and immunization documentation requirements for children under the age of 2.

The following inspection took place between the hours of 11:15am am through 2:00pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 6 children (17 points) with 2 caregivers. At departure there were a total of 7 children (19 points). A total of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in socially interactive activities in both English and Spanish. Inspector observed children play with toys, handwashing, during lunch time and naptime. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-920
Description: Based on observation, the menu posted did not document substitutions. Evidence: On the day of inspection, according to the menu, stir fry with tofu was to be served; however, children were served chicken nuggets, broccoli, rice and apples.

Plan of Correction: I will make sure to update the menu.

Standard #: 22VAC40-111-60-A
Description: Based on a review of records, 2 children's records were not maintained with up-to-date information. Evidence: The records for Child #2 and #3 were last updated 9/29/2016. An annual review was needed as of 9/29/2017.

Plan of Correction: I can have the parents sign today.

Standard #: 22VAC40-111-180-A
Description: Based on a review of records, 1 household member was missing documentation of a current report of subsequent tuberculosis screening [every two year]. Evidence: The last documented report of tuberculosis screening form for household member #1 was dated 3/26/15. An updated screening was need 3/26/17.

Plan of Correction: I will have him get the screening.

Description: Based on a review of records, 1 caregiver was missing documentation of orientation training. Evidence: Documentation of orientation for Caregiver #2 was not available at the time of inspection.

Plan of Correction: I will give her the orientation.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver records, central registry findings for a new employee were not obtained within 30 days of hire. Evidence: The record for Caregiver #2 was missing central registry findings.

Plan of Correction: I will send off the correct form.
