An announced inital inspection was began today at 10:30am. The physical plant, 5 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 4 children, totaling 10 points, under the supervision of 2 caregivers. During the inspection, children were observed coloring pictures, singing songs and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children will be served pasta with tomato sauce, chicken nuggets, grapes, strawberries and milk. The Provider is not certified in MAT (medication administration training), and there are no children with any medication needs. Should you have any questions, please contact me at

Description: Based on file review, 1 caregiver's record does not have all required information on file. Evidence 1. Caregiver #4 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment began.

Plan of Correction: The Provider will get two reference letters.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 4 out of 4 children's records do not have all required information. Evidence 1. Children #2, #3 and #4 do not have the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. 2. Children #2 and #4 do not have documentation of the policy number of their respective health insurance.

Plan of Correction: The Provider will ask the parents for this information.

Description: Based on file review, 1 caregiver does not have all required written documentation. Evidence 1. Caregiver #4 does not have documentation of having received orientation training.
