Technical Assistance:
Discussed handwashing, cleaning and sanitizing diaper changing station.

Comments:
An unannounced monitoring inspection was conducted today from 10:15am-12:15pm. There were 6 children (18 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, watching television and eating lunch. There was a supply of books, toys, and materials for the children. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on record review and observation, diaper ointments did not meet all requirements for a topical skin product. Evidence: Child D has a diaper ointment which is not labeled and does not have a parent authorization.

Plan of Correction: I will get the forms, fill them out, and get the signatures. I will put the child's name.

Standard #: 22VAC40-111-890
Description: Based on observation, food brought from home was not labeled with the child's name and date. Evidence: All food containers brought from home were not labeled with the child's name and date.

Plan of Correction: Ok, I will label.

Standard #: 22VAC40-111-940-A
Description: Based on observation, the temperatures in the fridge were not maintained at or below 40 degrees F and temperature in the fridge was not kept at or below 0 degrees F in the freezer compartments. Evidence: The refrigerator temperature when checked was 61 degrees F. The freezer temperature was 25 degrees F.

Plan of Correction: I might get a new fridge if it can't be fixed. (sic)

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's name and date. Evidence: All bottles in the refrigerator were not labeled with the child's name and date.

Plan of Correction: I will label with name and date.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain a complete record for each child. Evidence: 1) Child A is missing a work phone number for one parent and an address for a second emergency contact. 2) Child B is missing documentation of first date of attendance, medical insurance policy, verification of identity/proof of birth, and authorization of emergency care. 3) Child C is missing both parent work phone numbers, a second emergency contact, and documentation of first date of attendance.

Plan of Correction: I will check all the information with the parents.

Standard #: 22VAC40-111-70-A
Description: Based on the review of 3 records, the provider did not ensure parents received all required information before the child's first day of attendance. Evidence: 1) Child A, B, and C are missing signed documentation of the provider's medication policy.

Plan of Correction: I will have parent's sign the policy.

Standard #: 22VAC40-111-530-B
Description: Based on observation, cribs did not have a tight-fitting bottom cover. Evidence: Three cribs had crib sheets in place which were too big for the mattress in the crib therefore there was extra fabric which could cause a potential safety hazard.

Plan of Correction: We have a tight fittings sheet underneath. We removed the looser fitting sheet.

Standard #: 22VAC40-111-530-I
Description: Based on observation, a mattress was not covered with a waterproof material that can be cleaned and sanitized. Evidence: One mattress in a crib was ripped and torn exposing the interior of the mattrress and is no longer waterproof and cannot be cleaned and sanitized.

Plan of Correction: I will have to get a new one.
