Technical Assistance:
Discussed the background code changes effective 7/1/17. Discussed the anticipated requirement of fingerprinting of all staff and household members.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 5 children (18pts) with Provider and 1 assistant present today. Walk thru of programming area and observation of crib area conducted. A total of 5 children's records, all staff and household member records were reviewed. The home was equipped with ample toys, books and equipment that was accessible to the children who freely selected toys, puzzles and books. Staff observed reading books and playing a game where the children were instructed in spanish to find certain objects in the play area. Snack today was milk and cheese. Standard #: 22VAC40-111-100-A
Description: Based on child record review, one of five child records did not contain documentation of a physical exam. Evidence: Child #1 began enrollment on 8/7/17. No documentation of a physical exam was found.

Plan of Correction: I will request another physical exam from the child's parents.

Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records, two out of five records were not complete. Evidence: Child # 1 and #3's records were missing a second emergency contact.

Plan of Correction: Will obtain the second emergency contact from the parents.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on household record review, one background check was not obtained. Evidence: Household member #2 is 17 years old. A Central Registry search was not requested. Central registry search requests are required for all household members age 14 and older.

Plan of Correction: I will request this search this week and will forward the search request to the licensing agency within 10 days.
