Technical Assistance:
Discussed background changes that became effective 7/1/17 and upcoming fingerprint requirement with provider via telephone call. Provider was on a field trip with some of older children.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 7 children present(20pts) with 2 assistants. Provider was on a field trip with five of the children and spoke with Licensing Inspector via telephone call. Assistants present during the inspection presented knowledgeable of the programming, schedule and individual needs of the children currently in care. When the inspector arrived, children were finishing lunch. All children were strapped in their highchairs appropriately. A nice transition observed from lunch, to handwashing to free play. Staff were observed engaging with the children by playing with them and holding the younger children. Walk thru of home conducted to include the levels not used for programming. A total of 5 children's files, all household member and all staff files were reviewed today. No medications on site. It is the provider's policy not to administer medication. Observation of all four cribs and all cots used for sleeping was made. Thank you for your cooperation today. This inspection began at 11am and ended at 1:05pm. Standard #: 22VAC40-111-60-B
Description: Based on child record review, two of the five children's records were missing required information. Evidence: Child #3 and #5 records were missing the insurance policy number.

Plan of Correction: I will obtain this information from both children's parents.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, documentation for additional immunizations were not obtained for one out of five children's records reviewed. Evidence: Child #3 is 16 months old. the last immunization documentation on file is dated 8/16/16. Children under the age of 2 are required updated immunizations every six months.

Plan of Correction: I will obtain updated immunizations from the child's pediatrician.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on household record review, one household member did not obtain a required background check. Evidence: Household member #2 is 17 years old. No central registry search was maintained. Central registry searches are required for all household members age 14 and older.

Plan of Correction: I will request a central registry search immediately and forward the search request to the licensing agency within 10 days.
