Today an unannounced Monitoring inspection was conducted with Debra Goodman. This visit took place between 10:15am and 12:45pm. There were 10 children present with the provider and 1 assistant for a total of 25 points within the supervision guidelines. The children were observed singing songs, having circle time, doing the pledge of allegiance and eating lunch. Today's lunch is meatloaf, bread, green beans, applesauce and milk. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles were not labeled with the date. Evidence: At the time of the inspection, there was one bottle on the shelf in the infant area, one bottle on the counter in the kitchen area, and one bottle in the refrigerator. All three bottles were not labeled with today's date. Plan of Correction: We will do it ourselves.
Standard #: 22VAC40-111-100-A Description: Based on record review, children's records did not contain a physical exam within 30 days after the first date of attendance. Evidence: Child #2 file is missing a physical exam and their first date of attendance was 2/2/2017. Child #3 file is missing a physical exam and their first date of attendance was 1/19/2015. Child #5 file is missing a physical exam and their first date of attendance was 11/18/2013. Plan of Correction: I'll ask the parents as soon as possible tonight.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file is missing the child's address, parents names, address and phone numbers, a second emergency contact and a list of people authorized to pick-up. Plan of Correction: I'll give the parents a form today and ask for it back tomorrow.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain documentation of additional immunizations every six months for children under the age of two. Evidence: Child #2 date of birth is 10/16/2016 and their most recent immunizations on record are dated 12/19/2016. Plan of Correction: I'll ask for it and they'll bring them in as soon as possible.
Standard #: 22VAC40-111-280 Description: Based on observation, poisonous substances were not kept out of reach of children. Evidence: At the time of the inspection, there was a bottle of hand sanitizer on top of the book shelf next to the couch. There was a second bottle of hand sanitizer on the bathroom counter that the children use. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-111-680-B Description: Based on observation, children's hands were not washed prior to eating. Evidence: The children were sitting at the table playing games and singing songs. Lunch was served while the children were seated. The children's hands were not washed prior to starting to eat. Plan of Correction: We will have them wash their hands before snack today.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the Provider did not have the required central registry check. Evidence: The Provider did not have a central registry check on file. Plan of Correction: This has been sent out and has not been returned.
