Technical Assistance:
Reviewed all current and upcoming changes to background check procedures. All future information regarding background check procedures will only be communicated via email.

An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 11 children, totaling 32 points. Children's records, caregiver records, caregiver training, the physical plant, and emergency preparedness items were inspected today. The provider reported that no children's medications were on site today. This inspection took place in the middle of the day. The children were observed taking their afternoon nap. The children had been served salmon, rice, green beans, and milk for lunch earlier in the day. This inspection was completed between approximately 12:30 pm and 2:00 pm. Standard #: 22VAC40-111-800-A
Description: Based on a review of administrative documents, it was determined that the provider does not have all required emergency preparedness documents. Evidence: The provider has not created an Emergency Preparedness Plan.

Plan of Correction: I will complete the plan this weekend.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 and #5 do not include documentation showing that each parent has reviewed their child's contact information within the past year. 2. Child record #3 does not include: the address for one emergency contact person; a signed "Information for Parents" document; the provider's signature indicating that she has viewed the child's proof of identification; a signed "Provisions of the Emergency Plan" document. 3. Child record #4 does not include: a signed "Information for Parents" document; a signed "Medication Policy" document; the provider's signature indicating that she has viewed the child's proof of identification.

Plan of Correction: I will follow up with each parent for the missing information.
