An unannounced renewal study was conducted today beginning at 9:40am. 6 children (22 points) were directly supervised by 2 caregivers. Ratios were in compliance. The physical space, Emergency Preparedness Plan, Caregiver records, Household member records, children's records, evacuation drills and emergency supplies were reviewed. Required documents were posted. Children were observed finishing up with crafts, playing inside and outside, as well as eating. Diaper changing and hand washing procedures were also observed. The space was clean, organized and contained an abundance of age appropriate materials. Interactions between caregivers and children were positive. Standard #: 22VAC40-111-120-B Description: Based on record review, the assistant's record was incomplete. Evidence: The records for Assistant #1 did not contain the name, address and telephone number of a person to be notified in an emergency and did not contain two or more references. Plan of Correction: I will get the information today.
Standard #: 22VAC40-111-60-A Description: Based on record review, the provider did not maintain up-to-date records for 3 out of 4 children's records. Evidence: Child #1 and Child #2 were missing the medical insurance policy number. Child #3 was missing the medical insurance policy number and the Written Authorization for Emergency Medical Care. Plan of Correction: I will obtain the information right away.
Standard #: 22VAC40-111-200-A Description: Based on record review, the Provider did not obtain written orientation for the assistant by the end of their first week of assuming job responsibilities. Evidence: The assistant's first day was on October 30th and as of today there was no documentation of orientation. Plan of Correction: I verbally provided orientation but I will review and ask her to sign the document today.
