An unannounced monitoring inspection was conducted from 10:00 a.m.-11:30 a.m. with the provider. 12 children, 14 months, 18 months, 18 months, 2 years, 2 years, 2 years, 3 years, 3 years, 3 years, 4 years, 4 years, and 5 years old and one child, age 2 years, were under the care and supervision of two providers for a total of 29 points. Children were observed dancing, playing with puppets, painting a fruit and vegetables picture with watercolors, and eating morning snack. Required information was posted. The last emergency escape drill was 10/25/17. Both the provider and assistant hold current first aid and CPR certification. Two staff records and three children's records were reviewed. Standard #: 22VAC40-111-170-A Description: Based on review of two staff records, the provider did not have documentation of a tuberculosis (TB) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A?s (hired 8/28/17) record did not have documentation of a tuberculosis test or screening. Plan of Correction: Staff A did have a TB test. This documentation will be located and placed in her record.
