Technical Assistance:
During the inspection, licensing representative discussed with provider evacuation map requirements.

Comments:
The following inspection took place between the hours of 2:35pm through 4:15pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 2 children (5 points) with 2 caregivers. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were observed transitioning from naptime to toy play, reading and other activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of records, 1 child was missing documentation of a physical examination within 30 days of enrollment. Evidence: Child #2 [date of enrollment 3/6/2017] was missing documentation of a physical examination.

Plan of Correction: I will ask the parent for the missing information.

Standard #: 22VAC40-111-60-B
Description: Based on a review of records, 2 of 3 children's records did not contain the required information. Evidence: 1. Child #2 was missing the address for 1 of 2 emergency contacts. 2. Child #3 was missing medical insurance policy number.

Plan of Correction: I will ask the parents for the missing information.

Standard #: 22VAC40-111-210-A
Description: Based on a review of records, documentation of 16 hours of annual training was missing for 1 caregiver. Evidence: There was no documentation of annual training hours available for Caregiver #2 [date of employment May 2016].

Plan of Correction: Caregiver is in the progress of completing required training course online.

Standard #: 22VAC40-111-500-D
Description: Based on observation, S-hooks on swings were not tightly closed. Evidence: S-hooks of 8 chains [on 4 swings] were not tightly closed.

Plan of Correction: I will have them tightened.

Standard #: 22VAC40-111-530-A
Description: Based on observation, rest mats did not have proper linens. Evidence: 2 children were observed resting on mats without bottom linens.

Plan of Correction: I'll provide linens for the rest mats.
