Technical Assistance:

An unannounced renewal inspection was made today, 11/20/17, at 12:15 p. m. and concluded at 2:30 p. m. There were 12 children receiving care with points equaling 24 with two caregivers, later a third caregiver arrived. The children were observed having outside free play, lunch, story and nap time. The preschool is set up with a variety of toys and equipment for the children. The children sang Thanksgiving songs today. Lunch consisted of chicken nuggets, bun, tator tots, mixed fruit and milk. Five children records were reviewed. Three caregiver records were reviewed. Conversations were held with all caregivers and several children. No children on medications today. One caregiver is trained in MAT (medication administration trained). Classroom observations conducted in the preschool area. The last evacuation drill was documented on 10/24/17. Three shelter-in-place drills were conducted in 2017.

Description: Based on review of children records the provider did not have all requirements on file. Evidence: On 11/20/17, child #1 whose start date was 9/19/17 did not have a physical exam on file. Physical exams are required within 30 days of the first day of attendance.

Plan of Correction: The provider will ask parent for a copy of the physical exam for child #1.

Standard #: 22VAC40-111-210-B
Description: Based on discussion with the provider not all caregivers have received the required annual training. Evidence: On 11/20/17, one caregiver had not completed the required 16 hours of training.

Plan of Correction: Per provider, the caregiver will receive the required 16 hours for training this year.
