An unannounced monitoring inspection was completed on November 17, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. There were 5 children in care, for a total of 11 points with two caregivers. The children danced and sang songs, identified body parts and played until lunch time. For lunch, the children were served fish, string beans, mac & cheese, peaches and honey wheat rolls. The facility does not administer medication. The facility does not provide transportation. The last evacuation drill was conducted 10/18/17. The most recent shelter -in-place was conducted 10/18/17. Two caregiver records, and 4 children?s records were reviewed. Approximate time on site: 11:30am to 1:34pm
Standard #: 22VAC40-111-100-A Description: Based on a review of a sample of children's records, the facility did not ensure that a child's record contained documentation of a physical exam. EVIDENCE: The record of child#1, start date 9/12/17, did not contain a report of a physical examination. Plan of Correction: The mother will bring a copy of the physical examination today.
