The provider was given model forms today during the inspection and reviewed each one with the licensing inspector. The reqiuirements for children and home records for licensed programs are different from the ones required for permitted homes. It is recommended that the provider use the forms provided to ensure the provider has documented all required information
An unannounced monitoring inspection was conducted today, November 20, 2017 from 1:45pm until 2:50pm. There were a total of 5 children in care (14 Points) during the inspection. Children were observed to be napping and transitioning to from nap to a snack of crackers and applesauce toward the end of the inspection. Records were reviewed and model forms were provided to the provider to ensure all of the information required is collected. The home was clean and organized will an abundant supply of materials for children of the ages served.
Standard #: 22VAC40-111-920 Description: Based on observation, the menu for the current one week period was not available. Evidence: The menu posted was for the current day only. Plan of Correction: I will fix it.
Standard #: 22VAC40-111-960-E Description: Based on observation, bottlers were not labeled with the child's full name and the date. Evidence: Two bottles belonging to different children were observed without a label to identify the owner or the date. Plan of Correction: I will try and find labels that don't come off.
Standard #: 22VAC40-111-60-B Description: Based on record review, one child's file was incomplete. Evidence: Child 2's record did not contain the child's address, date of enrollment, the name, home address, and telephone number of each parent who has custody, name, office address and telephone number of the child's physician, the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached, Information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation, the child's health insurance policy number, proof of the child's age and identity and the names and addresses of previously attended child day care and schools as required by 22 VAC 40-111-80, and the parent's signed acknowledgement of the family day home's policies. Plan of Correction: I will make copies of the forms today and begin using them for all the children.
Standard #: 22VAC40-111-250-A Description: Based on observation, hanging cords were accesible to children under 5 years of age. Evidence: 1. In the childcare area a cord was plugged into the wall approximately 18 inches off the ground and followed the wall upwards and out the window. The cord was secured by the weight of the window sash causing the cord to remain taught and become a head entrapment hazard. 2. In the child care area an electrical cord for a phone was plugged into the wall approximately 18 inches off the ground and the cord followed the wall up to a ledge creating a situation where a child could pull the phone down onto themselves. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-111-530-A Description: Based on observation, cots used for sleeping did not have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. Evidence: Two children were observed sleeping on cots with only a blanket. Plan of Correction: I will put something.
Standard #: 22VAC40-111-530-G Description: Based on observation, one child under two was observed using a pillow. Evidence: Child 2 was observed sleeping in a crib with a filled pillow. Plan of Correction: I will talk to mom.
Standard #: VENDHOM-000-032-B Description: Based on absorption, children seated in high chairs did not have a protective belt fastened. Evidence: One child was observed without a protective belt fastened while seated in a table with seats for toddlers built into the surface. Plan of Correction: Corrected during inspection.
