63.2 Child Abuse and Neglect
Today an unannounced Monitoring visit took place in the morning between 10:20am and 11:45am. There were 7 children present with the provider and 1assistant for a total of 17 points within the supervision guidelines. The children were observed playing, being read to, washing their hands and getting ready for lunch. Today's lunch is spaghetti with meatballs and sauce, peaches and milk. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical exam within 30 days after the first day of attendance for each child. Evidence: Child #4 first date of attendance was 6/9/2017 and there is no documentation of a physical exam in their file. Child #5 first date of attendance was 9/20/2017 and there is no documentation of a physical exam in their file. Plan of Correction: I will talk to the parents when they pick up.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation of immunizations for all children by their first date of attendance. Evidence: Child #1 did not have documentation of immunizations. Plan of Correction: I will talk to the parents when they come to pick up.
