Today an unannounced Monitoring inspection was conducted between 10:45am and 1:00pm. There were 9 children present with the provider and 1 assistant for a total of 18 points within the supervision guidelines. The children were observed playing outside in the tree fort, interacting with the farm animals, eating lunch and beginning their nap. Lots of interactions with the children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 file is missing the child's first date of attendance and the phone number for their second emergency contact. Child #3 file is missing the child's first date of attendance and a list of people authorized to pick-up. Plan of Correction: I will notify the parents today and we will get it filled out.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review and interview, the provider did not have current certification in cardiopulmonary resuscitation (CPR). Evidence: The providers CPR certification expired 10/2017. The provider confirmed that her training had expired. Plan of Correction: I will check with red cross to find out when the next class is that I can take.

Standard #: 22VAC40-111-170-A Description: Based on record review, the provider did not obtain a negative Tuberculosis (TB) screening for all household members. Evidence: Household members #6 and #7 did not have negative TB screenings in their files. Plan of Correction: I will send them together to Patient First to get done.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review, the provider did not obtain a Sworn statement from all household members over the age of 18. Evidence: Household members #6 and #7 did not have Sworn statements in their files. Plan of Correction: I will have them do it today.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review, the provider did not obtain a Central Registry check for all household members over the age of 14. Evidence: Household member #4 did not have a completed Central Registry check in their file. The Central Registry check has been filled out by the household member, but it was not sent out for results. Household member #6 did not have a Central Registry check in their file. Plan of Correction: We will have the checks sent out.
