Technical Assistance:
Consultation was given regarding updated background check regulations and parent notification in event of an emergency.

An unannounced monitoring inspection was conducted today, November 20, 2017 to determine the provider's compliance with licensing standards. In attendance were 9 children totaling 26 points being supervised by the provider and assistant. Children were observed interacting with the providers, playing with toys, and eating. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home used by children were inspected with no hazards found. The family day home had the required postings displayed upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Four children's records and one provider's record were inspected with the majority of the required information found. The following was also inspected: fire drill log, emergency preparedness plan, parent agreements, and other pertinent information. See attached notice for violations found on this day. Standard #: 22VAC40-111-800-B
Description: Based on the inspection of the emergency preparedness plan, the required update was not completed. Evidence: During the inspection of the family day home's emergency preparedness plan dated 8/7/16, it was discovered the plan was not updated annually as required. The emergency plan should have been reviewed and updated in 8/2017.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-111-60-B
Description: Based on the inspection of four children's records the required information was not listed in the record. Evidence: During the inspection of four children's records, it was discovered that child #2 did not have the first emergency contact address listed; while child #4 did not have a second emergency contact listed. Licensing standards require two emergency contacts to be listed with the full street address and telephone number for each emergency contact.

Plan of Correction: Per provider: I will have the parents to write the information on the application during pickup time.

Standard #: 22VAC40-111-70-A
Description: Based on the inspection of children's records, the written information for parents that contain pertinent information was not in the record. Evidence: During the inspection of four children's records, it was discovered child #1 did not have the required written information for parents that contain pertinent information on the family day home. Licensing standards require this information to be provided to parents in writing before the child's first day of attendance.

Plan of Correction: Per provider: I will have the parent to review and sign the form and place it in the child's record.
