54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The family day home does not currently have children in need of medication, however the policy states that the family day home will administer medications to children if needed. On the date of inspection, neither the provider or substitute provider were fount to have current Medication Administration Training. Please wither locate the certificates, attend an approved Medication Administration Training or change the medication policy to reflect that the family day home does not administer medications.

An unannounced monitoring inspection was conducted today, November 20, 2017 from approximately 12:15pm-1:25pm. The substitute provider was observed caring for two children (6 points) during nap time. The provider is currently out of the country and the family day home has been operated by the substitute provider since Wednesday November 15th. The indoor home area was found to be organized and well supplied. The outdoor area is not being used by children as it is undergoing construction. An inspection of the outdoor area will be conducted when the construction is completed. An plan of correction was not discussed on this date as the provider was not available. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return to the licensing inspector by November 24, 2017. Description: Based on record review, an emergency evacuation drill was not practiced monthly. Evidence: There was no emergency evacuation drill documented for October 2017.

Plan of Correction: Drill record has been updated. Drills will continue to be documented.

Standard #: 22VAC40-111-140-D
Description: Based on observation and caregiver statement, the substitute provider does not record and sign the time of arrivals and departures on each day that she works. Evidence: There is no record in the family day home that shows the hours the substitute provider is working.

Plan of Correction: There is now an updated sign-in sign out form for the substitute provider.

Standard #: 22VAC40-111-200-A
Description: Based on record review, the provider did not orient the substitute provider and assistant by the end of their first week of assuming job responsibilities. Evidence: There was no record of Staff 2 having received an orientation by the end of their first week of assuming job responsibilities.

Plan of Correction: The record has been updated. The substitute provider is very experienced and knows expectations and responsibilities.

Standard #: 22VAC40-111-530-A
Description: Based on observation, not all rest mats used by napping children had linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. Evidence: Child A was observed sleeping on a rest mat with only a blanket.

Plan of Correction: New sheets have been made available for children and to comply with requirements. [sic]
