Discussed the renewal process and outlet covers.
Today an unannounced Monitoring inspection took place in this morning between 9:50am and 11:10am. There were 2 children present with the provider for a total of 7 points within the supervision guidelines. The children were observed being dropped off by their parents, napping and playing on the floor together. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Thank you for your cooperation. Standard #: 22VAC40-111-50-A Description: Based on record review and interview, the provider did not keep a written record of children in attendance each day. Evidence: There was no documentation of any attendance taken during the month of November. The provider states that she has not taken written attendance this month. Plan of Correction: I will record daily.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's files did not contain all required information. Evidence: Child #1 file is missing a second emergency contact and the child's medical insurance policy number. Child #2 file is missing the phone number for the child's physician, the phone number for their first emergency contact and the child's medical insurance policy number. Plan of Correction: I will ask the parents today for everything.
Standard #: 22VAC40-111-80-A Description: Based on record review and interview, the provider has not obtained proof of age and identity for two children. Evidence: There as no proof of identity for Child #1 and Child #2. The provider states that the parents have not yet brought them to her. Plan of Correction: I will ask the parents today for it.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation of immunizations for one child. Evidence: Child #2 file is missing documentation of immunizations. Plan of Correction: I have asked the parents for it.
Standard #: 22VAC40-111-210-A Description: Based on interview, the provider has not obtained the required annual training. Evidence: During the inspection, the provider stated that she has not attended any training this year. Plan of Correction: I will get the training.
