An unannounced complaint investigation was conducted today from 10:30am -11:20am, into allegations regarding a lack of supervision. Staff interviews were conducted. Due to the preponderance of evidence, this complaint was determined to be valid. A monitoring inspection and subsidy inspection were conducted on 4/7/17 as well; any violations unrelated to the complaint are identified in those inspections.
Standard #: 22VAC40-111-560-A Complaint related: No Description: Based on interviews, the staff failed to ensure direct care and supervision of a child to ensure awareness of and responsibility for their care. Evidence: On 3/22/2017, Staff #1 was alone in the home with Child #1 (6 months old), expecting the parent to arrive to pick up Child #1. Staff 1 unlocked the back door (main entrance to the childcare space), then went to the bathroom, leaving Child #1 strapped in a highchair unsupervised. The parent arrived to the family day home, walked in and found Child #1 alone and strapped in a high chair. The parent looked around the child care space for approximately 1 minute, did not hear or see a staff member, then took Child 1 and left the provider?s home. Staff 1 returned from the bathroom to continue caring for the child, however Child 1 was gone. Staff 1 had the provider call the parent, and the parent verified that they had picked up the child. Plan of Correction: The door will remain locked at all times and the parent will have to knock to enter the daycare. The door is usually locked and the only reason it was unlocked was to allow the parent to enter. The parent arrived about 20 minutes later than expected and caregiver forgot to relock the door before using the bathroom. Policy has changed. Now the door remains locked at all times and the parent has to knock to enter.
