Technical assistance today included crib sheets; sleeping patterns; record keeping.
An unannounced monitoring inspection was conducted today from 11 am - 2 pm. There were five children in care for 16 points. I observed lunch and preparation for nap. Standard #: 22VAC40-111-770 Description: Based on a review of emergency supplies today, there was no available battery operated weather band radio. Plan of Correction: I will get one.
Standard #: 22VAC40-111-100-B Description: Based on a review of records today, there was no physical exam for some children in care. Evidence: There were shot records but no physical exam for Child # 1, 3, 4, 5. Plan of Correction: I will talk to the parents for a physical examination.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records today, some information found to be missing. Evidence: There was incomplete physician information for Child #1; and no health insurance information for Child # 4. Plan of Correction: I will talk to parents today.
Standard #: 22VAC40-111-70-A Description: Based on a review of records today, there was not documentation that parents had been provided in writing all required information. Evidence: There was no complete medication policy for Child # 1, 2,3, 4, 5. There were no completed Emergency Procedures for Child # 1, 2, 3, 4. There was no signed Liability Insurance information signed for Child # 1, 2, 3, 4, 5, and the information for parents form written by the provider did not include information regarding the social services website; 24 hour policy regarding illness; daily schedule; pets in the home; discipline and guidance policy ; substitute information and other information. Plan of Correction: These will be completed.
Standard #: 22VAC40-111-80-B Description: Based on a review of records today, the provider did not verify the identity and age of children as required. Evidence: There was no signature and date of verification by the provider as provided, and no birth certificate number. as required Plan of Correction: I will look at the birth certificates, sign, write in the information and give it back.
Standard #: 22VAC40-111-240-A Description: Based on observation today, there were some areas of the children's area that were not maintained in a safe condition. Evidence: In the bathroom, the panels on the wooden vanity were observed to loose on one side. An outlet had a broken outlet cover accessible inside the outlet. Plan of Correction: My husband will fix the wooden part so it is always closed. I will cover the outlet.
Standard #: 22VAC40-111-320-G Description: Based on a review of records, observation and staff confirmation, there was no annual fireplace inspection or closing of the fireplace. Plan of Correction: I will close the fireplace.
