Technical Assistance:
During the inspection, licensing representative discussed with provider background check requirements for a household member turning 18 years old in the coming months, termination policies, and wood burning fire inspection to be conducted.

The following inspection took place between the hours of 12:30 pm through 2:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 2 children (4 points) with 1 caregiver. A total of 3 children's files and 3 caregiver/household member's files were reviewed. The children were observed engaged in various activities including outdoor play, imaginative play and arts and crafts. Inspector observed positive interactions, constructive guidance and redirection from provider. Handwashing and smooth transitions to naptime were also noted during this inspection. Description: Based on review, the emergency preparedness plan was not update annually. Evidence: The last documented review of the emergency plan was 9/30/2017. Documented review was needed on/prior to 9/30/2017.

Plan of Correction: I will complete the annual review.

Standard #: 22VAC40-111-100-A
Description: Based on a review of records, 1 of 3 children were missing documentation of a physical examination within 30 days of enrollment. Evidence: The record for Child #1 [date of enrollment: 9/25/2017] was missing a physical examination.

Plan of Correction: I will ask the parent for child's physical.
