54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted with the Provider. This visit took place in the morning between 11:30-1:00pm there were 2 children with the Provider for a total of 8 points within the supervision guidelines. The children were observed having morning lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance records, and emergency procedures were observed. Standard #: 22VAC40-111-60-B Description: Based on review of two children?s records, the provider did not maintain complete children?s records. Evidence: Child #1's records did not include both parents employers address and medical insurance policy number. Plan of Correction: Provider stated that she will ask the parent again today when she comes to pickup for the address.
Standard #: 22VAC40-111-210-A Description: Based on review of the Caregiver records, caregiver did not have a minimum of 16 hours of annul training. Evidence: Caregiver #1, record only had 12 hours of annual training, and was missing 4 additional training hours for 2017. Plan of Correction: Provider stated that she is planning to take additional training in the month of December.
Standard #: 22VAC40-111-240-A Description: Based on observation of the family day home, the home were found not being maintained in a clean, safe, and operable condition. Evidence: A square size hole was found along the wall near the steps to the childcare area. Chipped/peeling paint were found along the walls/door frame as you enter the kitchen. Plan of Correction: Provider stated that she will have the hole replaced and will have the walls repainted.
