An unannounced renewal inspection was conducted today from 10:10am to 12:25pm. There were 11 children (30 Points) in direct care of 2 caregivers. The children were observed morning snack brought from home. The children were engaged in small group activities including playing with toys, pretend doctor toys, home living, and the indoor play structure. The caregivers used positive redirection with the children. Hand washing was observed before lunch started. Emergency drills were maintained. Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's record was not complete. Evidence: 1. Child # 1's record did not have the physician's telephone number. 2. Child # 1's record did not have the medical insurance policy number.

Plan of Correction: I will get the information from the parents.

Standard #: 22VAC40-111-240-A
Description: Based on observation, inside areas of the family day home were not maintained in a safe and operable condition. Evidence: 1. Two areas of the ceiling in the day care area were missing ceiling tiles. The insulation and pipes are exposed to the children in the area. 2. Provider stated that the ceiling tiles are difficult to find. She also stated that it may take up to three months to replace the tiles.

Plan of Correction: Until ceiling tiles can be replaced, we will cover the openings with cardboard.

Standard #: 22VAC40-111-320-A
Description: Based on observation, small electrical appliances were not unplugged while not in use. Evidence: 1. A toaster in the daycare area was plugged in while not in use. 2. A toaster in the kitchen upstairs was plugged in while not in use.

Plan of Correction: I will make sure that small appliances are unplugged while not being used.

Standard #: 22VAC40-111-460-D
Description: Based on record review, the provider did not maintain documentation of the current rabies vaccination. Evidence: 1. The file for pet dog had documentation of rabies vaccinations that expired on 4/15/14. 2. Provider stated that the dog received vaccinations before Halloween. There was no documentation on file of the updated vaccinations.

Plan of Correction: I will put the vaccination records in the dog's file.
