- Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, the new definition of a serious injury, and a review of changes to background checks with regards to expiration dates, new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. Discussion that a CPS check for Member #2 sent back in September 2017 still has the status of pending within the Office of Background Investigations. A representative from that office advised that the Provider to send a new original request for Member #2 with a note stating that the original was never received back by the FDH. The Provider stated that she has called several times without any answer. New contact numbers were provided for the CPS office, along with this advise. The Provider stated that it shall be resent within the next 10 days.
The following inspection took place between the hours of 9:30 am through 12:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (20 points) with 3 caregivers. A total of 9 children?s files and 5 staff/household member?s files were reviewed. The children were engaged in the following: freeplay, toy play, center play within the FDH, play within the multi-purpose room, learning activities, coloring, games, music, song, diaper changing, bathroom breaks, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 9 children's file reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (11/29/2017), documentation of a physical examination for Child #8 was not available for review. 2. The start date for Child #8 was listed as 08/19/2017. 3. Child #8 was in attendance on the date of inspection. Plan of Correction: The Parent of Child #8 shall be contacted and a physical examination obtained.
Standard #: 22VAC40-111-200-A Description: Based on review, 1 of 5 staff/household member's files reviewed indicated that the documentation of an orientation training conducted was not available. Evidence - 1. On the date of inspection (11/29/2017), documentation of an orientation training for Member #2 was not available for review. 2. The start date for Member #2 was listed as 09/20/2017. 3. Member #2 was present within the Family Day Home on the date of inspection. Plan of Correction: This documentation shall be completed and placed on file for Member #2.
