54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today from 10:30am-12:30pm. There were 10 children ( 26 points) directly supervised by the provider and 2 assistants. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, working on an activity book, playing outside and eating lunch. Lunch consisted today of chicken nuggets, bread rolls, corn and apples. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-760-A-1 Description: Based on observation, the first aid kit is not complete. Evidence: The first aid kit is missing adhesive tape. Plan of Correction: I will buy more adhesive tape for the first aid kit.
Standard #: 22VAC40-111-60-B Description: Based on review of children's records, 2 of 5 children's records were missing information. Evidence: Child#1's record is missing documentation that a proof of identification was viewed within 7 days of the child's first day of attendance. Child#2's record is missing a second emergency contact and the street address for the first emergency contact. Plan of Correction: I will request information from parents.
Standard #: 22VAC40-111-320-B Description: Based on observation, an electrical outlet accessible to children under five years of age was missing a child-resistant protective cover. Evidence: One unused outlet in the programming area was missing a child-resistant protective cover. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review of three staff records and interview with the provider, the provider did not update a criminal history record report every three years. Evidence: Staff#1 has a criminal history record on file that expired on 1/28/17. Plan of Correction: I will submit a new form to the agency.
