Technical Assistance:
Please provide documentation that you have sent off for the background checks for Persons A, B and C by 12/18/17. Please provide documentation that you have obtained all of the required background checks for Persons A, B and C by 1/8/18.

An unannounced monitoring inspection was conducted today with the provider, who was present with 7 napping children and one assistant for a total of 22 points. No children had medication on site and their have been no injuries/accidents. Seven children's files were reviewed as well as three adult files. Arrival time: 1:40 pm Departure time: 3 pm

Standard #: 22VAC40-111-180-A
Description: Based on a review of the provider's files and an interview with the provider, it was determined that a person's chest x-ray/screening has not been renewed every two years. Evidence: Person B's chest-x-ray is dated June 2015.

Plan of Correction: I will have person A obtain a new screening.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a review of household members/assistant's files and an interview with the provider, it was determined that repeat background checks every three years have not been obtained. Evidence: Person A and B's criminal record reports are dated 8/20/14 and the central registry findings are dated 9/17/14. Neither have been renewed within 3 years. Person C's criminal record report is dated 1/15/13 and the central registry finding is dated 1/29/13. Neither have been renewed within 3 years.

Plan of Correction: I will send you documentation that I have already sent off for the background checks within 10 days and I will provide proof within 30 days that I have all the background checks back.
