A Subsidy Health and Safety monitoring inspection was conducted on 12/6/2017 to review supplemental health and safety requirements. This inspection began at 11:20am and ended at 2:00pm. There were 1 staff with 3 children at the time of the visit. At the start of the inspection, children were observed napping then having lunch. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were no medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Description: Based on record review, the provider did not document arrival and departure times of the children as required. Evidence: When reviewing attendance records, it was observed that the provider does not record the times that the children arrive and depart.

Plan of Correction: I will start doing that today.

Standard #: VENDHOM-000-012-A
Description: Based on record review and interview, the provider did not complete the required Virginia Pre-Service Training for Child Care Staff. Evidence: The provider did not have record of completing the Virginia Pre-Service Training. The provider states that she has not completed the training because she did not know the web site to complete the training.

Plan of Correction: I will do it.

Description: Based on record review and interview, the provider had not established a lockdown procedure. Evidence: The providers emergency preparedness plan did not include a lockdown procedure. The provider confirmed that she had not yet established a procedure.
